Who We Help
We’re here for adults who:
- Feel overwhelmed, stuck, or unsure what to do
- Are struggling with stress, anxiety, or emotional ups and downs
- Recently discharged from a psychiatric or substance use program
- Are transitioning from jail, prison, or re-entry programs
- Are waiting for services or experiencing a gap in care
- Don’t know what type of support they need

What We Do
We provide short-term, non-acute support for adults who need help now but do not require emergency or inpatient care.
Our focus is to help you:
- Feel more stable
- Reduce emotional distress
- Stay connected and supported
- Take the next step forward
Support is coordinated and personalized—you don’t need to choose services on your own.
A Bridge to What Comes Next
We don’t just help in the moment—we help you move forward.
As you begin to stabilize, we work with you to connect you to the next step in care, which may include:
- Ongoing outpatient providers
- Psychiatric care
- Inpatient or higher levels of care when needed
- Substance use treatment programs
- Sober living or recovery housing
- Re-entry and community support programs
- Other community resources
Our goal is to help you get where you need to go, not just where you are right now.
For Referrals
We welcome referrals from across the community, including:
- Hospitals and discharge planners
- Probation and parole
- Sober living and recovery programs
- Community organizations
- Individuals and family members
