Post-Discharge Crisis-Lite Behavioral Health Support

This referral pathway is designed for hospitals, emergency departments, and medical providers seeking rapid behavioral health follow-up after discharge.

Appropriate Referrals

Clients who are:

• Recently discharged from a hospital or emergency department
• Needing behavioral health follow-up within 24–72 hours
• Medically stable and not appropriate for inpatient psychiatric admission
• At risk for ED re-presentation without timely stabilization

What We Provide

• Crisis-Lite behavioral health stabilization
• Ongoing stabilization groups (rolling admission)
• Time-limited individual support
• Psychiatric evaluation & short-term medication stabilization
• Care coordination and warm handoffs
• Transportation support when clinically appropriate

What We Do Not Provide

• Inpatient or emergency psychiatric care
• Continuous monitoring
• Long-term outpatient psychotherapy
